    Documents required for opening bank account New customers are kindly requested to provide the documents listed here.
    Bank Account Agreement Both new customers and existing customers are kindly requested to use this document to specify needs for new accounts.
    Sub-account opening application Fill the application to open sub-account.
    Mandate to operate accounts If you want to authorize employees or other third party to make payment from your account, we kindly ask you to use this form.
    Password for communication with the bank ​This authorize the Bank to provide information about balance, account turnovers and transaction details on our bank accounts via telephone. 
    Declaration on beneficial owner Details of shareholders with a beneficial interest of the issued share capital of the company.
    Intended usage of accounts Details of anticipated account activity required in order to “Act on prevention of money laundering and terrorist financing”.
    Entity certification on FATCA status ​To be completed to identify new account holders in accordance with Polish Law on FATCA implementation as well as EU-directive 2014/107/EU and other acts of law issued in connection therewith.
    Depositor Information Sheet Depositor Information Sheet contains information regarding mandatory protection of deposits, required under relevant provisions of Danish as well as Polish law.
